Diabetes Education

Diabetes Educator

The community Diabetes Educator is available for one on one consultations for people with diabetes and impaired glucose tolerance. They play a key role in the management of an individuals diabetes by providing them with education on the fundamentals of diabetes, important components of effective management and education on how to measure and interpret blood glucose levels.

Effective diabetes management is a coordinated approach by the patient, their GP, diabetes educator, dietitian, podiatrist and optometrist or opthamologist. It is recommended that a person with diabetes visits their diabetes educator every 12 months to undertake and review their management.

The community Diabetes Educator is located in Vermont Street in the Community Rehabilitation Centre.

Diabetes Education Program

WRHS provides a five week activity based Diabetes education program.

During the program you will develop skills that will help you to self manage your diabetes.

Effective Diabetes management is essential if you wish to lead a happy and normal life. Understanding diabetes and how it affects your body is important.

The program is suitable for:

  • people with newly diagnosed Type 2 diabetes
  • people with impaired glucose tolerance
  • people who are having difficulty with effective management of their diabetes

The Program covers topics such as:

  • The Nuts and Bolts of Diabetes
    Basics of Diabetes management
  • Food Fabulous Food
    Relationship between food and diabetes, glycemic index and carbohydrates
  • Using the Right tools to Manage your diabetes
    Measuring and interpreting your blood glucose levels
  • Looking outside the kitchen we live in
    Supermarket shopping, eating out, take-away, label reading and fats
  • The Finishing Touches
    Inspect and Protect and introduction to foot care. The role medications play in diabetes management

Participants will need referral from their general practitioner and their latest pathology results.
